To receive credit for a teleconference viewed independently or for conferences viewed after the original air dates, you MUST fill out and return an evaluation form. This form may be emailed to Jessica Simpson or faxed to (520) 626-5652. CME credit for conferences viewed online will only be issued if completed within 3 months of the original air date.
